Hydroxychloroquine HCQ belongs to the group of antimalarial agents, which recognition of its benefit in other diseases except malaria dates back to 1894, when Payne reported that quinine was effective in cutaneous lupus. Since then, therapeutic effects of antimalarial agents were shown in a myriad of other diseases and the evidence for its potential benefit continues to grow.

Mechanism Hydroxychloroquine is thought to improve symptoms of systemic diseases by preventing inflammation. For example, to treat both rheumatoid arthritis and systemic lupus erythematosus SLE, it has been hypothesized that the drug suppresses T-lymphocytes by increasing the pH inside lysosomes, which disables leukocyte chemotaxis. The efficacy of antimalarials, especially hydroxychloroquine HCQ, in preventing systemic lupus erythematosus SLE flares is well demonstrated. However, many studies show that the percentage of SLE patients treated with HCQ remains low. Jan 01, 2020 Mechanism of action The precise mechanism by which hydroxychloroquine exhibits activity against Plasmodium is not known. Hydroxychloroquine, like chloroquine, is a weak base and may exert its effect by concentrating in the acid vesicles of the parasite and by inhibiting polymerization of heme.

If you've stopped taking plaquenil, did you start getting. I stopped my plaquenil on my own because I was getting severe muscle weakness An read that’s a severe side effect. Within a few weeks my flare up came back. My vitamin D was super low so I take 50,000 units An now felt better so I’m taking my plaquenil again. My muscle weakness was caused by low vitamin D.
