Dose of chloroquine at 4 0 year

Chloroquine has been extensively used in mass drug administrations, which may have contributed to the emergence and spread of resistance. It is recommended to check if chloroquine is still effective in the region prior to using it.

Results of one study indicate that chloroquine may exhibit nonlinear dose dependent pharmacokinetics. In this study, administration of a single 500 mg oral dose of chloroquine resulted in a peak serum concentration of 0.12 ug/mL, and administration of a single 1 g oral dose of the drug resulted in a peak serum concentration of 0.34 ug/mL. Hydroxychloroquine EC 50 =0.72 μM was found to be more potent than chloroquine EC 50 =5.47 μM in vitro. Based on PBPK models results, a loading dose of 400 mg twice daily of hydroxychloroquine sulfate given orally, followed by a maintenance dose of 200 mg given twice daily for 4 days is recommended for SARS-CoV-2 infection, as it reached three times the potency of chloroquine phosphate. Initial review of these data 6-9 suggested that retinopathy was rare if the total dose of chloroquine was less than 100 g. These symptoms are probably due to retinal degeneration, reported in 0.5 to 3.5% of patients treated with hydroxichloroquine 10-11 and in 10-25% of patients treated with chloroquine 12.

The Centers for Disease Control and Prevention recommend against treatment of malaria with chloroquine alone due to more effective combinations. In areas where resistance is present, other antimalarials, such as mefloquine or atovaquone, may be used instead.

  4. Apr 02, 2019 Some advocate high-dose diazepam e.g. 2 mg/kg over 30 min, followed by 1-2 mg/kg/day for 2-4 days post-intubation in the patient with severe chloroquine toxicity. This stems from the French experience e.g. Riou et al, 1988 which suggests much improved mortality in patients receiving high dose diazepam.

Jan 18, 2013 Chloroquine 2013-01-18 Reactions 760 - Complete heart block 2 case reports Complete heart block developed in 2 women after they received long-term high-dose treatment with chloroquine for rheumatic diseases. The first patient, aged 73 years, took chloroquine 250 mg/day for 12 years for rheumatoid arthritis. Chloroquine is also a lysosomotropic agent, meaning it accumulates preferentially in the lysosomes of cells in the body. The pK a for the quinoline nitrogen of chloroquine is 8.5, meaning it is about 10% deprotonated at physiological pH as calculated by the Henderson-Hasselbalch equation. This decreases to about 0.2% at a lysosomal pH of 4.6. For Child 3–4 years. is unlikely if the dose of chloroquine phosphate does not exceed 4 mg/kg daily equivalent to chloroquine base approx. 2.5 mg/kg daily.
