    The risk of a toxicity sharply increases after 5 years, with majority of cases of retinotoxicity occurring in patients that have had a cumulative dose exceeding 1000g of hydroxychloriquine Plaquenil. When SOME patients first start taking Plaquenil and take 2+ tablets from the beginning, sometimes this can affect the eye muscle. When Plaquenil is stopped, the symptoms disappear. This is why many lupus specialists begin with 1 tablet a day 200 mgs and then increase to 2+if necessary. I was started on 200 mg BID so a 400 mg dose daily. I have not had any issues with my Plaquenil and I am thankful because it has helped so very much with the joint pain as I am not able to take NSAIDS due to my kidneys. I have not been 100% since before the worst of it all, but I have many better days where the joint pain is quite minimal.
